  • Aberdeen City Health & Social Care Partnership (ACHSCP)

    ACHSCP emerged in February of 2016, and is now responsible for a variety of services across adult health and social care. GCC work collaboratively with the Partnership, as we deliver care and support at home within Aberdeen City.

  • Care Management Referral Form

    Aberdeen Care Management provide help to adults of all ages who experience difficulty coping with everyday activities due to disability, illness, and/or the ageing process.   • Aberdeen Guide to Independent Living & Enablement (AGILE)

    This information booklet is for those that are interested in finding out what help is available in Aberdeen to those that wish to remain as independent as possible in their own home, but require support to do so. There are a variety of resources available that do not require professional referral; these are free of charge and may be just what you are looking for. To find out more, click the link below.

  • Technology Enabled Care (TEC)

    TEC is a term used to describe telehealth, telecare, mobile electronic and digital health and social care services. This has come into focus over the last two years, with significant challenges such as staff retention and recruitment presenting a need for alternative service delivery. With this, a plan has been developed by ACHSCP to put these digital services into action.   • Self Directed Support (SDS) Handbook

    SDS is the name for how people in Scotland source care and support. This process was introduced in order to give the public control and flexibility over the care they receive. While Granite Care Consortium provide care and support under option 3, we appreciate that there are four different options which you can choose from.   • Bon Accord Care

    While Bon Accord Care (BAC) is not a member of GCC, we work alongside them to meet the needs of supported people in Aberdeen. BAC services include occupational therapy (OT), telecare services (to access equipment such as community alarm, fall detector, automatic lights), equipment, and wellbeing activities and information.

  • Aberdeen Vaccination and Wellbeing Hub

    This Vaccination Hub is located inside of Aberdeen’s Bon Accord Centre, and is host to a variety of different health and social care services/projects.
